Key Accountabilities
∙ To provide support , to disabled young people in need and their families in line with statutory requirements and departmental priorities, assessed needs and the joint
AHCW / SCF Transitions protocol..
∙ To ensure that work is focused on achieving positive outcomes for young people and work with them, using a person centered approach to create a support plan which will facilitate choice and empowerment through promoting independence as far as possible.
Main duties
Qualifications/ Experience
∙ DipSW, CWSQ or equivalent professional Social Work Qualification.
∙ A practical understanding of physical, emotional and attitudinal barriers facing young
disabled people.
∙ A good understanding of social care organisations, its role and responsibilities.
∙ An understanding of the various frameworks such as the LSC, and SEN, around the support of young people with complex needs.
Key skills
The transition process is a critical time for young people with additional needs and their families. It marks the move from school to adult life and from children's to adult's services. Many young people, particularly those with additional needs are likely to require continuing access to a combination of support services in order to enjoy a good quality of life in Essex adulthood.
