Consultant-Transitions form Children to Adult Services

My client a local authority is looking for a consultant to look for the development and presentation of a written strategy for the transition form Children's to Adult Services.

Main duties

Main responsibilities will include:
- specifying arrangements to transfer individuals from children's to adult's services across a range of provision,
- identifying the preferred indicators to measure the success of the above processes
- review and rewrite the existing documentation in the light of comments from key agencies and individuals

Key skills

The person completing the work will report to a nominated senior manager and through that manager to the Transitions Group chaired by the Director of Adult Services
